PAYMENT POLICY

Peak Physical Therapy & Wellness is in-network with the following health insurance companies:

• Aetna

• Anthem

• AultCare

• Cigna

• Medical Mutual

• Medicare

If your insurance company is listed above, we will collect your insurance information at the initial evaluation, verify outpatient physical therapy benefits, and submit claims directly to these companies. Any portion not covered by insurance during the plan of care (i.e. co-pays or deductibles) will be billed to the patient.

If your insurance company is out-of-network with our clinic or you are part of a health share (i.e. Christian Healthcare Ministries, Amish Church Fund, Wayne Holmes Mutual Aid), we ask that you pay a self-pay rate at the time of service and we will provide you with an itemized receipt that you can then submit to your specific company.
